I tried to select "Workshop Moms/Pops" using the radio button, and I got an error from Google Docs. It looks like all of the other volunteer options use check boxes, not radio buttons. Perhaps if you change the type of control to a check box for Workshop Moms/Pops then it will work properly. -Christin The form that is used for volunteering is broken.
